HEADER - Service Provider

Mystic Valley Elder Services contracts with service providers (other organizations/companies) who work directly with our community members and program participants. The Executive Office of Elder Affairs determines the types of services that Mystic Valley can offer. The Executive Office of Elder Affairs is responsible for approving agencies interested in providing Homemaker/Personal Care Services. For more information, visit their website at 800ageinfo.com.

There are numerous documents forms for review during every RFP process. We’ve packaged (zipped) the files by category, which will allow you to download many files at the same time. These files are in Word Document (.docx), Word Document Fillable Form (.docm), and Adobe Acrobat (.pdf) formats. If you do not have Adobe Acrobat or Adobe Reader on your computer, click the icon below to download it.

The contract procurement application process has closed for FY 2018. The next procurement will be conducted in 2021. If you would like to be notified when the procurement process commences, send an email to the contracts manager at [email protected].
