At Mystic Valley Elder Services we provide practical solutions to everyday living. We work with older adults, families, caregivers and health care professionals to identify and access the many services and programs that allow elders to remain safe and independent in their homes. We are a one-stop resource for elder care information, advice, services and solutions for anyone… regardless of age or income. Many of the services we offer are available to elders and families in our service area at low or no cost.
Since 1975, Mystic Valley Elder Services has been a leader in the field of elder care. As one of the largest aging services organizations in Massachusetts, we serve the communities of Everett, Malden, Medford, Melrose, North Reading, Reading, Stoneham and Wakefield.

As Case Manager you will provide assistance to low income elders and disabled adults. Your responsibilities will include determining program eligibility for your clients and acquiring services that will help them continue to be able to live independently in the community. You will determine your clients’ eligibility by assessing their needs with a combination of home visits and telephone contact. You will also be responsible for monitoring your clients’ on-going services.
This is full -time benefits eligible position.
A Bachelor’s degree in social work or a related field. Strong interviewing and assessment skills. Strong interpersonal and organizational skills. Excellent written and verbal communication skills. Proficient with Microsoft Office Suite and entering narrative and other data into a database. Ability to use the Internet to conduct information searches. Private vehicle. Preferred requirements: 2 years of experience working with elders, experience working in a community setting. Social work license preferred.
