The primary responsibility of the Home Care Care Manager, within the framework of the ASAP regulations, determines program eligibility and provides a client centered approach to assessment, service acquisition, reassessment, and monitoring of the services provided, to assist elders to live independently in the community. The Home Care Care Manager conducts their assessments and monitoring by a combination of telephone contact and home visits.
This is a full-time (36 hours per week) position.
A Bachelor's degree in social work or related field. Must have strong interviewing and assessment skills, and strong interpersonal and organizational skills. Proficient with basic Microsoft Word, Microsoft Outlook and entering narrative and other data into database. Ability to use the Internet to conduct basic information searches. Private vehicle. Knowledge of and experience working with older people, and experience working in community setting preferred. Social Work license preferred.
