memory disorder can be one of life's most painful challenges.
Facing this challenge alone, without guidance or assistance, makes
it doubly hard.

Memory Disorder Care Planning consultation includes: |
- Discussion
of caregiver concerns
- Home
assessment emphasizing safety and other risk issues
- Up-to-date
information on memory disorders, including Alzheimer's
Disease

- An
individualized care plan, including recommendations regarding
services, home safety, community resources, managing behaviors,
and more
- Follow-up
phone calls and/or visit(s) to assist in setting up the
care plan
|
The Memory Disorder Program gives families struggling
with this situation a helping hand when they need it most. By
offering education, support, and practical suggestions about managing
care, the staff of the Memory Disorder Program enables families
to provide the best possible care at home for the longest time
possible. Master's level clinicians provide memory disorder consultations
according to the certification standards of the Alzheimer's Association
of Massachusetts.
