Long Term Care Assessment

The Long Term Care Assessment is available to families and individuals considering nursing home care. A registered nurse experienced in both nursing home care and other community and in-home care options performs the assessment.

Screenings for nursing home placement are available at no cost to individuals with Medicaid.
