For an elder or disabled adult living at home, a Care Manager can:

• Conduct an in-home needs assessment in consultation with caregivers
• Research the best available care options that fit within the family's resources
• Assist in vendor selection and service arrangement
• Monitor quality care and report to the family
• Assist with selection/transition to an assisted living or nursing facility
• Provide specialized consultation and training on the care of someone with Alzheimer's Disease or other dementia disorders.
• Serve as a liaison with health and social service providers
• Escort individual to medical appointments and report back to the family
• Make referrals for legal, financial, or mental health needs

For an elder or disabled adult living in a facility, a Care Manager can:

• Consult with facility staff regarding a suitable care plan
• Monitor the quality of care through regular visits
• Report to the family on changes in an elder's status or care issues
• Serve as liaison with facility staff, elder, and physician
• Assist with Medicaid applications and other paperwork
• Coordinate services upon hospital discharge
• Consult with facility staff regarding special issues of elders with Alzheimer's Disease or other dementia disorders.

Senior Options Care Mangers are seasoned professionals who are credentialed in nursing or social work. Most are active members of the New England Chapter of the Association of Professional Geriatric Care Managers. Mystic Valley Elder Services is a nonprofit organization that has no ownership interest in any direct service or facility. Senior Options Care Management services are billed at an hourly rate.
