Senior Options Care Management

Whether you live nearby or at a distance, caring for an elderly or disabled relative can be difficult, stressful, and time consuming. A professional Care Manager can offer expertise, advice, and access to services that can enable an older or disabled person to age with dignity and give you peace of mind.

For an elder or disabled adult living at home, a Care Manager can:

  • Conduct an in-home needs assessment in consultation with caregivers

  • Monitor the quality of care through regular visits
  • Report to the family on changes in an elder's status or care issues
  • Serve as liaison with facility staff, elder, and physician
  • Assist with Medicaid applications and other paperwork
  • Coordinate services upon hospital discharge
  • Consult with facility staff regarding special issues of elders with Alzheimer's Disease or other dementia disorders.

Senior Options Care Mangers are seasoned professionals who are credentialed in nursing or social work. Most are active members of the New England Chapter of the Association of Professional Geriatric Care Managers. Mystic Valley Elder Services is a nonprofit organization that has no ownership interest in any direct service or facility. Senior Options Care Management services are billed at an hourly rate.
